Since 2009, Professor Yablonsky has been the Director of the Saint-Petersburg State Research Institute of Phthisiopulmonolgy – the oldest center for tuberculosis control in the Russian Federation. Under his leadership, the Institute became a base for developing guidelines and regulatory documents defining the main directions for reforming and developing the country's phthisiatric service. The Institute has reformed its scientific and personnel potential, reconstructed the existing clinical and laboratory base, commissioned a new clinical building, and equipped it with the most modern equipment, allowing it to address priority scientific problems and provide high-tech care. Professor Yablonsky implemented, for the first time in world practice, the concept of using robot-assisted surgeries in the treatment of patients with pulmonary tuberculosis. To date, the Center for Thoracic Surgery has accumulated the most extensive experience in Russia in performing robot-assisted surgeries (more than 1000) for various diseases of the thoracic cavity organs.

The priority research direction is the application of thoracic surgery methods in the pulmonary tuberculosis and other localizations treatment. For the first time in the world, hybrid operations were performed on patients with active pulmonary tuberculosis and dissecting aneurysms of the thoracic and abdominal aorta, and on patients with a combination of tuberculosis and lung cancer using artificial circulation. A world priority is the studied and scientifically substantiated effectiveness of robot-assisted surgery technology in the tuberculosis patients’ treatment; personal experience exceeds 500 operations. Noting the international authority of Professor Yablonsky, it should be mentioned that in 2010, at the proposal of the WHO European Office, he headed an international group of experts from 10 countries, the result of which was the adoption of the WHO Consensus on the role of thoracic surgery in improving the effectiveness of treatment of drug-resistant pulmonary tuberculosis in 2014.
The scope of scientific interests of Professor Yablonsky is not limited to thoracic surgery. He is also actively developing a fundamental direction of scientific research. Under his leadership and with his direct participation, experimental research is being conducted to find new anti-tuberculosis agents, develop candidate vaccines, cellular products and nanostructured materials for the treatment and rehabilitation of tuberculosis patients, and preclinical evaluation of their effectiveness has been carried out. The created museum of Tuberculosis cultures allows studying the influence of genetic mutations of mycobacteria virulent properties changes and the mechanisms of drug resistance formation. The study of lung parenchyma destruction molecular processes using advanced computer technologies of spatiotemporal modeling has made it possible to determine prognostic factors for the development of tuberculous inflammation and identify the mechanisms underlying its progression and reduction. The results of these developments have been published in leading world publications and are protected by invention patents.
Professor Yablonsky’s sphere of scientific interests includes modern aspects of cardiothoracic surgery, pulmonology and phthisiology, and the problem of lung transplantation. In 1990 he was one of the organizers of the city Center for harvesting, preservation and tissue typing of donor organs, and for the first time in Saint-Petersburg, he introduced the method of multi-organ procurement from donors with confirmed brain death.
The fundamental mechanisms of lung protection from ischemia, the features of performing isolated lung, both lungs, heart-lung complex, trachea transplantation operations were studied. In 1999, he defended his Doctor of Medical Sciences dissertation: "Transplantation of the isolated lung (experimental and clinical study)" - the first in Russia. In 1991, the world's first successful simultaneous transplantation of the thoracic trachea was performed, and in 2006, the first successful simultaneous transplantation of both lungs in the practice of Russian healthcare.

Under the guidance of Petr Kazimirovich Yablonsky, 25 Candidate of Sciences and 2 Doctor of Sciences dissertations have been successfully defended.
The results of Professor Yablonsky scientific research are published in the most recognized scientific journals in Russia and abroad, making him one of the most cited cardiothoracic surgeons in Russia. In the ranking of Russian scientists, Professor Yablonsky is in the top 1% of authors with the highest number of citations, and according to the Science Index, he is included in the first percentile of the RSCI core. The Hirsch index for publications in the Russian Science Citation Index (RSCI) is 30, in the international Scopus database – 19. Today, his bibliographic list includes more than 1000 scientific works. He is a co-author of 75 intellectual property objects, including 31 invention patents, 44 databases and computer programs.
The most significant works are considered to be those on the diagnosis and treatment of acute and chronic lung diseases, reconstructive and plastic surgery of the trachea and bronchi, publications on the role of invasive research methods in modern pulmonology and differential diagnosis of lung lesions, research on the problem of isolated lung transplantation, and the search for new diagnostic methods and drugs for the tuberculosis treatment.
